APPLICATION PROCESS

Completed application should contain:

- Online submission form completely filled out
and submit to faculty sponsor
- Faculty sponsor approves by clicking on the submit to UCUR button

***Submit application no later than November 17, 2006.

!!!! Not all e-mail programs recognize the file
or function correctly:
If the form does not automatically attach to a new email, then the form is not correctly functioning.  If this is the case, then fill in the form, print the form, have your faculty sponsor sign the form, and then deliver the hard copy to your institutional contact. These alternative steps will be necessary for Hotmail, Yahoo, and any other web-based email clients.
